Soccer (Volleyball) Registration Form

WE WILL PROVIDE this form on the day of tournament. This form will need to be filled out again on the day of tournament. The reason is, a lot of teams have changed their players. In order to keep the most updated list of players in a team, this form will be filled out again on the day of tournament. This form is what we will use to check players' names, shirt ##s, and etc.

This form becomes very important as the teams move up and compete for semi-finals or finals. We will use this form to check if teams change their players to win the tournament, or if they are still playing in the same team.

In the past, players changed team in the middle of the game to win the championship. It is against the rule, and the teams will be fined up to $250 for doing so.
